What Matters To You, And What You Can Expect From Us.
You can ask the care manager for a weekly schedule of your calls so you know who is coming and what time
Your calls will be on time and for the duration on the care plan
If care workers are going to be more than 15 mins earlier or later than expected the care manager will call you and make it right
If something goes wrong or there is an emergency at a previous call the care manager will let you know straight away and will make alternative arrangements for you
If you need to talk to the care manager about anything all you need to do is tell one of your carers. They will pass on your message. You can also find her contact number in this client file.
The care manager will visit you as often as you need her to and she is always available to answer your questions. She is in charge of your care.
The team are committed to making sure your care package runs smoothly
Primecare wants you to feel part of making decisions so will ask you to complete a questionnaire once a year to collect data on how we are doing or if we need to do better.
If things change the care manager will review your care and update your care plan. She will also tell all the care staff of any changes
If you wish Primecare to manage your medication, the care manager will arrange to deal with all aspects of your medication. You don’t need to worry about a thing.
If you wish to manage your own medication then that is ok too.
Primecare staff are very careful about your belongings and we will do everything we can to keep your things safe. However, we cannot be responsible for any accidental breakages.
What we expect from you:
To feel that you are at the centre of the care we provide to you.
To ask us questions if you have any questions or concerns.
The care manager, you and your family will set up your care plan together. Once completed we all work together to make sure it is working and that you are happy with it. Please tell us if you are happy or, tell us if something is not working.
It is your responsibility to maintain appliances in your home.
We ask that you and your family comply with your plan.
Please do not to ask the care workers to do things that are not on the care plan. If you need something adding then the care manager will review your care.
It is your responsibility to make sure our care workers are safe and not put at risk from poor lighting, heating or objects, slippery or icy surfaces. If you cannot do this then the care manager will help you. Just call and she will come and see you.
You must make sure you have adequate insurance to meet these needs.
This next one hardly ever happens but we have to say it: Primecare has a zero tolerance for any form of abusive language or aggressive behaviour of any kind. This will result in an immediate withdrawal of care provision and the local authorities will be consulted as to what happens next.
It is your responsibility to make sure that you and any person coming into your home has a safe exit in case of fire. The fire service recommends all homes are fitted with heat and smoke detectors. If you do not have these, tell us and we will arrange for someone to fit them.
