Primecare (North Wales) Ltd is a private independent provider of domiciliary care for people who require support within their own homes or within the community.
Primecare (North Wales) Ltd is based in Llandudno and we have been providing services in North Wales since 1999. We are a small team who offer homecare services to people in our local community.
The services that we provide are from between 1 to 24 hours and 365 days a year. This support maybe purchased by you in full or part or from the local authority/social services.
Primecare (North Wales) Ltd is registered by Care and Social Services Inspectorate Wales (CIW) and listed as a Private Provider with Conwy Social Services.

Our staff and home carers are trained to the highest standards, and are friendly, caring and reliable. I am proud to say they always give 100% and that although we are small in comparison to other agencies we are mighty in our commitment to quality care provision.
All our staff must undergo rigorous training and induction courses before being sent into your home. In this, you can be assured of safety, discretion and reliability. All staff are formally checked by the police and DBS to guarantee that you can be certain of personal safety.

Our service provides a person-centred approach to all your care requirements.
Primecare (North Wales) Ltd offers a wide range of services to young or old people who have a learning difficulty, elderly, elderly mentally infirm, a physical disability, people with a terminal illness or anyone who may need care and support in their own home or in the community.
We can offer the following services:
DOMESTIC SUPPORT
Primecare (North Wales) Ltd can provide support and assistance with housekeeping, cleaning, meal preparation, shopping and laundry duties.
CARE WORKERS
Our qualified care workers provide a helping hand with a wide range of domestic duties. Among these are: housekeeping, cleaning, meal preparation, shopping and laundry services and personal care. We do not clean windows, move heavy furniture or climb ladders.
MOBILE SUPPORT FOR LEISURE ACTIVITIES
Clients needing a more mobile service can rely on Primecare (North Wales) Ltd to provide the mobility to take the client out and about. The client may need to visit the hospital, G.P. or pursue personal activities with the assistance of our staff.
PALLIATIVE CARE
Primecare (North Wales) Ltd supports the client throughout the varying stages of illness. We can offer a complete package of care and will work alongside with your G.P. District Nurse or other professional care setting.
NIGHT SERVICE
For those clients who do not wish to be alone or need reassurance during the night, the sleeping service may be of use. Primecare (North Wales) Ltd can also offer and awake night service to an individual person in their own home.
THE RIGHT PACKAGE OF SERVICE FOR YOU
The care and support for any individual who wishes to make their own choices, is of the utmost concern to Primecare. We offer a 24 hour, 7 days a week, 365 days a year support service.
Primecare (North Wales) Ltd will liaise between G.P. District Nurses, Community Nurse Teams, Hospital Trusts and Social Services. The Primecare Care Manager is there to manage the care package and to assist in managing an individual’s household and therefore maintaining their choices and independence. Support can be provided on a short or long-term basis.
If you wish to go ahead with a package of care from Primecare (North Wales) Ltd; just give us a call and our Care Manager, Susan Mortimer will visit you, and together with you and your family, assess your needs. This visit will be free of charge.
Following this assessment, all parties should have a clear understanding of the type of care you will need Primecare (North Wales) Ltd to provide. A care plan will be set out by you and this will be followed by our care staff when they make their calls to you. The care plan sets out clear guidelines and tasks. The tasks are laid out in a clear and concise way so everyone knows what to do on every call and you can be assured that all your agreed needs are being met.
The care plan will be available for you to review, in the client file. This is a book which our care staff use in your home.
Primecare (North Wales) Ltd has the flexibility to meet any changes that may occur in your future requirements. Should you need to change your care plan the care manager will visit you again. The care manager is responsible for ensuring the care package runs smoothly and you are happy with the care we provide to you.

A National Assembly regulatory body that determine whether an Agency provides professional, competent and means tested standards of care when delivered in their own home. CIW monitors all care providers in Wales.
